ALTERNATE PROFESSIONAL DEVELOPMENT MODEL

 

 

The Alternative Professional Development Model Forms are available by clicking on the link/icon located on bottom of this page. 

 

  1. Please print this page before opening the form.
  2. Read all directions before proceeding.
  3. Open the Microsoft Word Format form needed by clicking on the icon.
  4. SAVE the form with a new name immediately (before you have entered any data). 
  5. SAVE your own work on your H: drive, hard drive or floppy disk.

 

To begin using the Alternative Professional Development Model Forms:

  1. The cursor will be positioned in the first blank of the form.
  2. Type in your work, then Tab over to the next blank on the form.
  3. You will continue using the Tab key after each entry.
  4. The box entries require an “X”.  Type in the letter “X” or right click.
  5. Continue entering data and using the Tab key until all sections have been completed.
  6. If you wish to go back to a previous entry, use the UP () arrow key, reposition the cursor, or use the Page Up and Page Down. 
  7. A word of caution… In the boxes, they will continue to grow if you type more than the allotted space.  If you need additional space, use a second page.
  8. When you are finished entering the information you desire, save it on your hard drive or floppy disk.
